These forms describe more fully who we are and what we do, what you can expect in terms of our communication and conduct, and help you facilitate an expedited check-out.

We really appreciate it when new clients take the time to download, print, read and sign the forms and bring them with them to their first appointment. It not only facilitates the process and saves you time in our waiting room, it also gives you an opportunity to learn more about us and ask questions prior to your first appointment.

If your first appointment is a remote appointment by Skype, iChat, or telephone, please return your signed forms prior to the appointment date.

Forms can be scanned in and emailed to info@alphagammabrain.com. If you don't own a scanner, we recommend taking a digital photograph, which will work just as well so long as the resolution is clear.
